Abstract

The Homewood Home Visit program is a student-led initiative providing care coordination, longitudinal follow-up, social needs screening, and food/produce delivery for high-risk, low-income, and racially/ethnically diverse patients after discharge. This study used student feedback to evaluate medical student education through the home visit program, underscoring its importance in experiential learning.
