Abstract

The CATCH program uses tele-outreach to schedule annual well-child visits (AWCVs) for patients lost to follow-up and offers access to community-based resources. This study assessed CATCH’s efficacy in scheduling AWCVs and providing resources and found increased rates of AWCV scheduling and access to local resources to address unmet social needs.
